Ms. Sims is available for your new patient inquiries Monday through Friday, 8:00 a.m. - 4:00 p.m. at (504) 648-2530. When scheduling a new patient appointment with IMS, please have the following information available for her:
Ms. Sims will do an initial intake with you and schedule your appointment. In some instances it may be necessary for her to consult with the physician to determine the appropriate timeframe during which your appointment must be scheduled. You can either request to have a new patient packet sent to you or you may download the forms from our patient forms tab and print them out. If you choose to download the forms please fill out the following:
Our office will call two days prior to your appointment confirm your attendance. To expedite your registration process, please arrive at least 30 minutes before your scheduled appointment time and bring all forms with you. Please DO NOT MAIL the forms back to IMS. In addition, you will need to bring the following information with you: · A photo ID · Your most current insurance card(s) · A referral form if required by your insurance plan · Your co-payment if required by your plan · A list of current medications and how you take them (some physicians require that you bring in all of your medication bottles in for review. Ms. Sims will instruct you if this is necessary.) · Copies of any recent test results that pertain to your appointment Patient Parking is available by parking meter, off street parking or in the parking garage connected to our building for a cost. When you get to our office, you must check in with the front desk receptionist and present your insurance card(s) and ID. It is the sole responsibility of the patient to provide IMS with all insurance information and referral from your primary care physician. If you do not have a referral and one is required, your appointment will be rescheduled unless one cannot be immediately obtained. Your insurance company gives this office the responsibility of collecting co-payments at the time of service. We do not bill for co-payments. Your co-pay must be paid in full at the time of service. Co-payments are collected at check-in. If no insurance is given at the time of service, then you will be responsible for the entire bill. A payment of $150.00 will be collected upon check-in. Any over payment based on services performed will be refunded at the end of the visit. Any additional fees incurred due to services performed will be collected at the end of the visit. We accept cash, check, MasterCard, Visa, American Express, and Discover. If you are unable to keep your appointment due to your schedule, please cancel or reschedule at least 24 hours in advance to allow us the courtesy of offering your appointment time to another patient. If you are late, the provider will decide whether or not to see you. If the provider approves seeing you, there may be an additional wait time.
